SecureCollect™ Instructions

SecureCollect™ is the Chemisys specimen integrity program — designed to protect every sample from the moment it leaves the patient to the moment it arrives in our laboratory. These instructions walk clinical staff through proper collection, labeling, and shipping.

Before collection

• Confirm the test order and required specimen type.
• Verify patient identity using two identifiers per your practice protocol.
• Prepare a SecureCollect™ kit and confirm the seal is intact.

During collection

• Follow the collection method indicated on the requisition (urine, oral fluid, blood, hair, or nail).
• Label the specimen container at the point of collection — never in advance.
• Document the collector, date, and time on the chain-of-custody form.

Sealing & shipping

• Place the specimen inside the tamper-evident SecureCollect™ bag and seal completely.
• Attach the completed requisition to the outside pouch.
• Store per test requirements and ship using the Chemisys-provided courier or shipping label.
